If you'd like to arrange for a one-time $25 contribution deducted from your Local dues check, fill out the Milkshake Man Fund Deduction Authorization and mail, fax (202-434-1481) or e-mail it to:  cwamail@cwa-union.org

(Note:  Dues check deductions are not available to Locals that are 100% local collection or cash dues.)
